Semaglutide is designed to mimic some of the biological activity of natural GLP-1 while remaining active in the body for longer. Depending on the licensed product and medical indication, semaglutide may be supplied as a weekly injection or an oral tablet.

How Does Semaglutide Work?

2. Clinically Significant Weight Loss

Semaglutide has produced meaningful weight reduction in clinical trials involving adults with obesity or overweight accompanied by qualifying health conditions.

In the STEP 1 trial, adults without diabetes received once-weekly semaglutide or placebo alongside lifestyle intervention. After 68 weeks, the semaglutide group experienced an average body-weight reduction of approximately 14.9%, compared with a much smaller change in the placebo group. Around 86% of participants receiving semaglutide lost at least 5% of their starting weight.

Individual results vary considerably. Some people lose more weight, while others experience a smaller response or stop treatment because of adverse effects.

Semaglutide is therefore best understood as a treatment that may support weight management when combined with nutrition, physical activity and ongoing clinical assessment—not as a replacement for these factors.

Weight reduction can positively influence several measurements associated with metabolic health.

Clinical studies of semaglutide have reported improvements in areas such as waist circumference, blood pressure, blood-lipid measurements and markers of glucose regulation. The size of these changes varies according to the population studied, the formulation used, treatment duration and individual response.

Some benefits may be partly related to weight loss, while others could result from GLP-1 receptor activity itself. It would therefore be misleading to promise that every user will experience the same metabolic improvements.

6. Greater Mobility and Physical Function

For people living with obesity, weight reduction can sometimes make everyday movement and exercise more manageable.

A clinical trial involving participants with obesity and knee osteoarthritis found that semaglutide treatment, together with lifestyle counselling, resulted in greater weight reduction and improvements in osteoarthritis-related knee pain compared with placebo.

Semaglutide is not a direct treatment for every type of joint pain. However, reducing excess mechanical load may improve comfort and mobility for some people whose symptoms are affected by body weight.

7. Emerging Benefits for Liver Disease

Metabolic dysfunction-associated steatohepatitis, known as MASH, is a progressive liver condition involving fat accumulation, inflammation and liver scarring.

This development demonstrates how semaglutide research has expanded beyond weight management and diabetes. It should not, however, be interpreted as evidence that semaglutide is appropriate for every person with fatty liver disease.

Is Weight Loss Maintained After Semaglutide Is Stopped?

Semaglutide does not permanently remove the biological factors that can contribute to obesity.

In the extension of the STEP 1 trial, participants regained a substantial proportion of the weight they had lost after semaglutide was withdrawn. Several cardiometabolic improvements also began moving back towards their original levels.

This suggests obesity may require continuing management rather than a brief treatment followed by no further support.

Long-term nutrition, resistance training, physical activity, sleep quality and behavioural strategies remain important. Anyone considering stopping prescribed semaglutide should discuss an appropriate maintenance plan with their healthcare provider rather than stopping or changing treatment independently.

Does Semaglutide Only Reduce Body Fat?

Weight loss caused by a sustained calorie deficit usually includes a mixture of fat mass and lean tissue.

Semaglutide does not automatically preserve muscle. A person eating very little protein, remaining inactive or losing weight rapidly may also lose meaningful amounts of lean mass.

Where medically appropriate, a structured resistance-training programme, sufficient dietary protein and a sensible rate of weight reduction may help support muscle retention. These should be adapted to the person’s health, ability and clinical circumstances.

Common Side Effects

The most frequently reported adverse effects are gastrointestinal and can include:

GLP-1 medicines should not be used during pregnancy, while trying to become pregnant or while breastfeeding. Current UK guidance recommends stopping semaglutide at least two months before a planned pregnancy.
